Said applicant(s) attest that the information provided on this application is true and accurate to the best of my/our knowledge.
Said applicant(s) understand and agree that completion and submission of this application does not guarantee that I will receive a rescue pet to foster.
Said applicant(s) understand that any misrepresentation of fact will result in the removal of the foster dog from my/our home.
Said applicant(s) understand/agree that in the event we wish to adopt any ASIAN DOG RESCUE animal I/we will be required to follow the same adoption process as required by all individuals requesting to adopt an animal from our organization and will be required to pay said normal stated adoption fee.
Said applicant(s) understand and agree that any animal we foster is solely "owned" by Precious Paws Asian and Small Breed Dog Rescue and upon demand for return of any fostered animal by said rescue that I will immediately make animal available or such refusal will constitute theft and I/we will be charged accordingly. I/we will be responsible for all legal fees incurred by rescue organization to recover rescued foster pet.
Said applicant(s) understand and agree that I/we need expressed approval before I/we can take foster animal to any vet for any reason unless a life threatening issue or prior agreed upon weekly veterinary/grooming appointments arranged by ADR for long term fosters and special needs pets.
I/We understand all pets in my foster care will be given top quality veterinary care by ADR veterinarians prior to my fostering. If due to distance between myself, foster and ADR veterinarians, other arrangements will be made by ADR representatives for vet care in my area and I will be responsible for ensuring foster animal immediately gets said veterinary care and grooming arranged by ADR and at ADR's expense.
I/We am responsible for administering any medicines, special foods, etc. (provided by ADR) that my foster(s) need to ensure their continued health and well being.
I am responsible for bathing, brushing and keeping my foster pet's coat mat free and clean. I will keep foster pet(s) eyes and ears clean and free of debris while in my care and in between professional grooming appointments.
I/We will notify an ADR representative immediately upon noticing any change in my foster pet(s) health, attitude, eating habits, bowel habits or any sign of lethargy, distress or change in general demeanor.
Said applicant(s) understand and agree that if rescue pet gets harmed, sick while in my care due to negligence on my part, that I am fully responsible for all bills to make said rescue pet healthy again.
Said applicant(s) understand and agree ADR has no history on any animal I may foster and ADR will not be held liable shall I get bit, injured or if foster animal in any way harms another person/animal while in my care.
Said applicant(s) understand and agree any destruction to property/furnishings by foster pet is my sole responsibility and ADR will not reimburse any costs for said behavior. Foster pet is my sole responsibility while in my care and I will take full responsibility for it's actions.
