Precious Paws Asian Dog Rescue Adoption Application

Please read carefully. This is a legally binding contract once you adopt your pet. All applicants are screened for suitable placement of our animals.

PLEASE READ OUR ADOPTION POLICIES BEFORE EMAILING US WITH "GENERAL" QUESTIONS.

If you still have questions please take the time to read our ADOPTION CONTRACT , which should answer any questions that you may have concerning our adoption policies or your responsibility as a new adopter of one of our rescue babies. You will be required to sign it and adhere to our regulations.

OUR ADOPTION POLICIES

Thank you for considering adopting a rescue pet from us. If you are interested in adopting one of the precious animals we have rescued, you must fill out an adoption application, submit to a home visit if asked and be approved. Once approved you must pick up your new pet as soon as possible to make room for our next rescue which is most likely coming from a kill shelter and whose life depends upon us rescuing it. These animals are "rescues". They have been possibly abused, but always discarded by their previous owners. You will need to be tolerant and loving while they adjust to their new home which can take a month or more. Please note that we can never say with complete certainty that our babies are 100% potty-trained. We are always hesitant to claim any pet is completely housebroken as we want you to expect to have to reinforce training in your own home as the babies initially adjust to a new atmosphere. Mere nerves can cause an otherwise very well housebroken dog to have many accidents in a new home for the first few days/weeks. There is always a period of adjustment, and you must be willing to work with your new family member. We do train them here to be wee-wee pad trained and/or outside potty-trained, but you must expect to have to reinforce this training, or continue training in your home.

The adoption fee is non refundable, whether you keep your adopted pet one hour or one year. Thankfully we get VERY FEW returns. We screen our adopters carefully and we expect your decision to adopt to be thought out carefully and we want you to realize that your commitment is for the life of the dog. Our adoption fees vary. Please read more about our ADOPTION FEES . We are a "hard rescue organization" which means that we accept dogs with serious medical needs. In addition to treating the usual minor medical conditions, vaccinating, and altering, we also arrange for more expensive procedures such as heartworm treatments, orthopedic surgeries, and ophthalmic care. In order to pay for this expensive care, we must ask for an "adoption fee". Occassionally, certain breeds and/or extremely excessive costs involved in the rescue and/or care of a particular dog or many dogs needing surgery in our rescue at one time may cause our adoption fees to be higher than we normally would ask for. The adoption fee helps defray the vet costs of the dog you are inquiring about and the extremely high veterinary bills that are incurred by the many animals we save. We are a small rescue, we have no grants or funding. The time and amount of money it takes to make these pets the beautiful, adoptable and socialized babies you see is extensive. We also have "special needs" dogs that require continual and specialized veterinary care. Our dogs are always altered, have all vaccinations and heartworm tests, and are always on heartworm and flea and tick preventative. Many times they have been professionally groomed several times and had various other costly medical procedures. We are not a "public shelter". We are a group of foster homes. Unfortunately we do not have public facilities and are unable to let people "come see all of the dogs". You must fill out an adoption application and be approved before you will be able to meet the pet you have inquired about adopting. We do this to be certain that you are an appropriate home for the animal so as to not waste anyone's time. Once you are approved for adoption, a volunteer will pick up your dog from it's foster home on day of adoption and meet you at a public location. We do this for the safety and convenience of all parties involved. When you adopt a baby from here, you also save the life of one who is scheduled to die in a kill shelter, because you have opened up a space for that new baby to come here. This is also why we request you to pick up your new baby, if approved, ASAP.

We have an "adoption committee" that reviews applications and approves each adoption. We receive many applications on each dog and the adoption process is not first come, first serve. If we have several qualified applicants, we simply choose the best home for that particular dog in our opinion. Volunteers answer emails. We are all volunteers with families and jobs. We do this out of our love for the animals and receive no financial compensation.

Thank you for applying to adopt one of our precious furangels. Please think out your decision to adopt a particular animal CAREFULLY The animal's welfare is our top priority. We do strive to make certain the adopter is happy, we want both pet and owner to be happy and find their forever friend for life. If the adopter isn't happy, then the pet won't be and we have failed everyone involved. We reserve the right to decline adoption of any animal to any person for any reason.

We are having some problems with our application delivery system. Please ALWAYS follow up your submission of an application with an email to us to make sure we receive it. If possible, copy/paste a duplicate of your application after you have filled it out for a back up in case we do not get the first one.

I agree to show proof of this to any representative of Precious Paws Asian and Small Breed Dog Rescue if asked.

I agree that this pet will always be kept current on all licensing requirements by the state and city in which I live. I agree to show proof of this to any representative of Precious Paws Asian and Small Breed Dog Rescue if I am asked.

I agree to provide quality, name brand food and fresh water 24 hours a day for this pet.

I agree that this pet will be strictly and exclusively an indoor family pet and will not be used in any way detrimental to him/her; such as for medical research or in any way that could cause hardship or pain to the dog.

I agree to provide daily exercise for this dog under my supervision and will not allow him/her to run free and never chain this dog.

I agree to be responsible for any property damage from this day forward that this pet may incur.

I agree to notify Precious Paws Asian Dog Rescue if animal comes up missing or stolen or confiscated by the authorities.

I agree that this pet will remain in my custody and possession. I will not sell, trade, give away, transfer ownership or dispose of him/her, including taking this rescue pet to any other rescue organization or shelter. Pet must be returned to Precious Paws if not in my possession.

I may return this rescue pet at any time during it's life to the rescue organization, but I understand that I will not be refunded my adoption fee. I understand that Precious Paws Asian and Small Breed Dog Rescue will not refund any costs; such as veterinary expenses, license fees, or the cost of pet supplies. I agree that I am hereby responsible for any and all costs incurred by this animal from adoption day forward.

I understand that most rescue pets do not come with a history and I will not hold said rescue responsible, liable for any mistake in the interpretation of age, temperament or training level. I understand this pet has been seen by a veterinarian, however,I understand and agree that Precious Paws Asian and Small Breed Dog Rescue can not be responsible for the physical condition or temperament of this dog, nor can they guarantee the pet is housebroken.

I am aware that this pet has been in the custody of Precious Paws Asian and Small Breed Dog Rescue due to cruelty, neglect, injury or abandonment prior to being rescued. It may take several weeks-to-months for him/her to adjust to their new home, including housebreaking just as with any animal.

While in my care, this pet will be given an annual vet check, along with vaccinations, and maintained on a quality heartworm preventative/flea/tick preventative for the rest of the dog's natural life.

If I ever decide I don't want this animal anymore it MUST immediately be returned to Precious Paws Asian and Small Breed Dog Rescue at my expense and time with no refund of adoption fee. If there are no foster homes available, I agree to keep the pet I adopted inside my home, safe, well cared for and happy until a foster home opens up or another adopter is approved.

No refund of adoption fees once pet is adopted under any circumstances. After paying an adoption fee this fee is immediately absorbed into the rescue and is unable to be refunded.

Think out your decision to adopt carefully. It is a LIFETIME COMMITMENT.These pets are not perfect. They are a work in progress. Any animal brought into your home(regardless of where you get your new pet from) will require additional work on your part. As with anything living, constant care, attention, training and reinforcement is required for their entire life.

PLEASE ANSWER EVERY QUESTION THOROUGHLY AND EXPLAIN YOUR ANSWER/REASONING WHEN ASKED.

No human is perfect, nor is any animal. If you will be upset if a new dog has an "accident" on your floor or tries to jump on your couch or growls if a child pulls it by the tail across the floor, then please don't apply to adopt one of our animals. These animals are "rescues". This is NOT "Perfect Pet Adoption Center". They have been possibly abused, but always discarded by their previous owners. You will need to be tolerant and loving while they adjust to their new home which can take a month or more. The adoption fee is NOT REFUNDABLE. If you choose not to keep the pet at any time, you MUST return the pet to Precious Paws Asian and Small Breed Dog Rescue with NO REFUND. If we are full and do not have room at the time you wish to return the dog, you MUST keep him at your home and safe until we have an opening or find him a new adopter. We require updates periodically via email with pictures. Do you understand and agree to these things?

How did you hear about Precious Paws Asian and Small Breed Dog Rescue?(ex:friend, other rescue, one of our adopters)Please be specific.

Please provide the name, address and home phone number of two (2) references and their relationship to you. No family references please.

Please read the following carefully. Also PLEASE send a separate email letting us know you have submitted an application to make certain that we receive it. Some applications have been lost. Please save a copy of your completed application for a back up in case this one gets lost.

I declare that the information I have provided in this application is complete and correct. I further declare that I am financially and physically able to care for a rescued dog, I understand that proper food and veterinarian care may be costly, and I am able and willing to meet the needs of my adoptive dog. I submit that if the Adoption Application or the home visit either before or after adoption reveals falsification or misrepresentation of any facts on my part, along with not adhering to ANY of the above agreed to policies, Precious Paws Asian and Small Breed Dog Rescue reserves the right to either refuse adoption or to nullify the adoption and reclaim said rescued dog from my premises. I understand the adoption fee/donation must be submitted in cash only. I understand it can take a month or more for my adopted friend to adjust to their new surroundings and any pets I may have. This is a serious decision and we will not refund adoption fees to parties who choose to back out on their adoption contract after they have taken possession of the dog. We do this for several reasons. We hope it will make people think long and hard before adopting one of our babies. These dogs do not deserve to be handed back and forth. Once you adopt we expect this to be the baby's last, permanent home.
